RELATIONSHIPS WITH COMPENSATION COMMISSIONER:
Employee:
It is the duty of the employee to:
- report incident within 12 months (with medical reports).
Employer:
The employer has to fill in the following
- WCl 1 (E) - Report of occupational disease.
- WCl 14 - Notice of an occupational disease.
- OD2 - Industrial Hearing Loss Exposure Form.
The following supporting documentation has to be provided by the employer:
- the history of the employee,
- the period of exposure,
- the daily duration of exposure,
- whether the employee is still exposed,
- when was the employee last exposed,
- the noise levels dBA,
- the baseline audiogram,
- 2 diagnostic audiograms,
- a copy of the employee’s ID.
Duties of OHMP/ENT:
They must submit, via employer:
- WCl 22 (First & Final Report)
- Medical reports (all other causes excluded).
Administrative challenge:
The following information needed for each employee:
- Surname, Names, ID, DOB, age, Clock #.
- Employer (code), department (Code).
- Date of employment.
- Current & Previous Noise levels and Exposure (dates).
- Hearing Protection (Y/N/Sometimes) - since when?
- Medication (ototoxic).
- Colds, Allergies, Wax.
- Family history.
- Head injury.
- Ear discharge / infections.
- Previous diseases (TB, meningitis, etc.).
- Tinnitus.
- Military Service; Hobbies.
- Physical Examination.
- Baseline audiometric data and PLH.
- Current hearing threshold levels and PLH.
- Difference between baseline & current - relevant comments.
- Details of actions.
- Names and codes of OHMP, audiologist.
- Medical Reports.
- Archive old audios (40 years).
- New Noise Level (noise zones, levels, exposure).
