This is a periodic screening audiogram of a employee who has had a baseline audiogram performed before the cut off date of 16 November 2003. The shift from baseline is 6,8% - thus not compensatable. There was a remarkable shift in the 3kHz - 6kHz frequencies, indicating noise-induced hearing loss. This employee should be reported to the Inspectorate of Labour with noise-induced hearing loss, but not to the Compensation Commissioner.
